- 1. Fully active, able to carry on all pre-disease activities without restriction
- 2. Restricted in physically strenuous activity, but mobile and able to carry out work of a light or sedentary nature, e.g. light housework, office work
- 3. Mobile and capable of all selfcare but unable to carry out any work activities; up and about more than 50% of waking hours.
- 4. Capable of only limited selfcare; confined to bed or chair more than 50% of waking hours
- 5. Completely disabled/immobile; unable to carry on any selfcare; totally confined to bed or chair

Care Oncology – Schedule and Expedite Initial Consultation and COC Medications

Many of our patients have requested to bypass the patient coordinator call and proceed with scheduling with a COC Oncologist to more quickly receive the COC Protocol. Many of these patients have been referred by their oncologist, have read the many books that recommend Care Oncology or have done their research and made a decision to move forward. If you are ready to move forward, please proceed below. At any time, you can select to meet with a patient coordinator with further questions.

Payment for your Initial Consultation includes the time with a licensed Care Oncology Oncologist as well as 3 months Oncology Nurse Support.
