Health History & Biomarker Panel
During the Onboarding Process we ask patients to:
- Complete a comprehensive questionnaire to learn about their health history, cancer diagnosis and lifestyle habits.
- Provide medical records around their cancer diagnosis, current treatment plan and any recent blood work to inform our oncologists.

How long does the onboarding process take?
Information submitted by you – the onboarding questionnaire and your baseline medical records – is promptly reviewed by our team. So, the sooner we receive these documents, the sooner we can order your custom blood panel and schedule your initial oncologist appointment.
