Tell Us About You

Fill Out A Patient Profile Form

Fill out a profile form prior to our meeting online. This is done securely on our site so that we can get to know your situation a bit better before speaking.

Call with Your Case Manager

A 30-Minute Conference

This call will last about 30-minutes and is dedicated to learning more about you and your unique situation. Any requested support providers, family, and friends can attend as well, just let us know.

This call is to review your case, go over the Care Oncology Protocol, and find out how to best proceed.

Medical Review

Send Us Your Medical Records for Review

We will assist you in obtaining any records and supporting information relevant to your situation.  Here are the Medical Records your Care Oncology Oncologist and Oncology Nurses need for review.

All records are then peer-reviewed, and we will send available times to meet with a Care Oncology oncologist.

Confirm Your Appointment Time

Secure and Confirm the Appointment Time Setup with our Scheduler

You will receive instructions with a secure link and all information necessary to attend your appointment with our physician.  This 1 minute video shows how our HIPAA compliant telehealth video system works.  You can test your audio and video prior to your consultation with the Care Oncology Oncologist.

Speak with Our Nurse

A Meeting with an Oncology Nurse to Get You On Board

You will have a meeting with one of our Oncology Nurses who will get all necessary information for the Care Oncology oncologist.

As a Care Oncology patient, you will have ongoing access to our Oncology Nurses to ask questions, schedule phone calls and video consultations.

7.
Meet with Our Oncologist

Attend Your First Meeting with our Care Oncology Oncologist

Your consultation with the Care Oncology oncologist will include a review of your medical records, discussion of the COC Protocol and how best it can help in the context of your specific situation and current treatments.

Consultations generally last 45 minutes to 1 hour so you’ll have plenty of time to ask the COC oncologist questions.

8.
Treatment Plan

Receive Your Customized Treatment Plan and Get Your COC Protocol

You will receive detailed information about your Care Oncology treatment plan, along with the next steps to obtaining your medications through our U.S. partner pharmacies. All medications will be shipped directly to you.

You will continue to have access to ongoing support with our nurse oncology team throughout the entire process.

9.
Video Conference Follow-up

Begin Your Treatment Plan and Follow-up in 90 Days

Once your Care Oncology treatment plan begins you will need to follow-up every 90 days with new blood work (CBC, CMP, and recent scan reports if available). Your Care Oncology oncologist will review your standard-of-care treatment and decide on any changes to your customized COC Protocol.

10.
Ongoing Support

Help Every Step of The Way

Our outstanding Oncology Nurses are available for ongoing support between meetings with your Care Oncology oncologist. You’ll have the option of email, scheduled phone calls and even scheduled video meetings with our nurse team. Care Oncology staff will help with all steps prior to meeting with the oncologist, including obtaining medical records.
