COC Plus Enrollment & Payment
How to Enroll for COC Plus Program, and what is included:
1. Step One: Complete the basic enrollment form on this page so we know how best to contact you throughout your participation in the program.
2. Step Two: Secure enrollment in the program through the payment form. The cost is $330 USD payable via credit card or paypal.
3. Step Three: Check for your payment confirmation and Health Survey link. Complete this survey at your earliest opportunity as it will form the basis for assessing your current health status and factors affecting your metabolic pathways and markers.
4. Step Four: Schedule your blood draw for the metabolic test panel. You will receive an email when your lab order has been placed along with an appointment link and directions. Please note, cost and payment for the blood draw is your responsibility and not included in the program fee.
5. Step Five: Meet with a Care Oncology oncologist. Once we receive your test results, our team will schedule your comprehensive consultation that includes a detailed evaluation of your current health status and outline an intervention strategy.
6. Step Six: Receive your Personalized Health Plan created by the oncologist.
Care Oncology has been treating patients since 2013. Our oncologists have experience consulting with many patients with different cancer types. They will work alongside, and in coordination with, varied standard-of-care regimens being overseen by your existing healthcare team.
