  • Communicate with patients’ primary care providers and specialty doctors; support patients and families through complex palliative care treatment; support appropriate transfer to Hospice level of care, collaborate with and supervise interdisciplinary team members.

  • Oversee primary medical clinic, Haitian staff, and supervise rotating clinic coordinators.

  • Founded and implemented a new Palliative Care program in conjunction with Holyoke Medical Center Oncology Department.
  • Responsible for admission, development of plan of care, case management, and appropriate transfer to Hospice for a large caseload of Palliative Care patients in Holyoke and the surrounding communities. Supervised home health aides, translators and volunteers.
